A new study has found that about two-thirds of health care systems are using population health management programs as a way to address readmission, patient education, and acute care. Additionally, more than half of the health care systems have programs in place to support patients living with chronic medical conditions. However, researchers believe that care coordination continues to be a top challenge for these organizations.
In a survey of 105 healthcare IT leaders, 23.5% said that care coordination was their organization’s biggest challenge when it comes to effective population health management. Other challenges included financial investment (21.4%) and data management (18.4%).
