In many integrated health systems, home care is becoming an increasingly important component. Now, changes in Medicare reimbursements are further prompting healthcare systems to boost their home care services. This change could affect the bottom line using cost avoidance measures like reduced admissions.
Hospital systems that have embraced the idea of adding more homecare services have seen benefits to both costs and patient health. For example, Allegheny Health Network’s Healthcare@Home program helped the system to reduce all-cause 30-day readmissions in 2015 by 5%. Additionally, the readmission rate for high-risk patients, including those with congestive heart failure, was shown to have dropped by 8%.
